Report a data breach

This form must be completed by any member of staff who suspects or becomes aware of a data breach. Submit the completed form to your designated contact as instructed in the Data Breach Policy. Do not attempt to resolve the matter yourself, and do not contact the Information Commissioner's Office or any affected individuals.

You may submit this form anonymously. If you wish to do so, select anonymous reporting below and do not include your name or contact details in Section 1.

Where a breach is likely to result in a risk to individuals' rights and freedoms, the business is legally required to notify the Information Commissioner's Office without undue delay, and in any event no later than 72 hours after becoming aware of the breach. Prompt reporting of suspected incidents is therefore essential.
